Choose an amount of $500, $250 or $100 and half of your donation will automatically go to UH Seidman as a 100% tax-deductible gift. The remaining half will be put on a gift card for you to use at a participating Five Star restaurant. Simply choose a restaurant from the list below and the gift card will be mailed to your home.

Does my gift card have an expiration date?: Each restaurant will be utilizing their own gift cards. Please check your card upon receipt for the expiration date.
Will I get a tax receipt for the UH Seidman Cancer Center portion of my transaction?: Yes. Half of your online credit card transaction will be donated directly to UH Seidman Cancer Center and is considered 100% tax-deductible. All purchasers will receive both a confirmation email and a letter via USPS to reflect this. Exact breakdowns are as follows:

How will I receive my gift card?: Your gift card will be mailed to the address you provided during checkout. Purchasers should receive their gift card 5-7 business days after purchase via USPS.
