UH Select brings together thorough primary care, personalized wellness and a team that keeps everything connected in one membership.

UH Select is University Hospitals' concierge primary care program. An annual membership fee of $2,900 per person allows doctors in the program to spend more time with each of their patients. The program, open to anyone age 3 and older, allows members to enjoy same- or next-day appointments with a member of the UH Select team, a dedicated patient navigator available around the clock and direct access to their physician.

Each year, those enrolled in UH Select receive a thorough physical exam with routine bloodwork and lab tests, along with regular check-ins to stay ahead of health concerns. Members also choose among nutrition guidance from a registered dietitian, a fitness evaluation through UH’s partnership with T3 Performance or access to wellness services at UH Connor Whole Health, including massage therapy and acupuncture.

UH Select also offers members a dedicated clinical pharmacist to review medications, a health coach and a behavioral health coordinator to manage care alongside a UH Select physician. When members need specialty care, the UH Select team helps coordinate visits and follow-up across the UH system.

UH Select has locations in Beachwood, Fairlawn, Mayfield Heights, Solon and Westlake at UH St. John Medical Center. In April, the Westlake location welcomed Jason Frazier, DO, with Cynthia Caja, DO, starting in June, making care even more convenient for West Side patients.
